Outline of Implementation

Please be sure to register on KENPOS. Please note that all items that do not go through KENPOS appointments are fully copayments.

• * For those who have newly joined our health insurance society, for example by joining the company, it will take about one month before they can use the system.
• * Please complete your initial registration for KENPOS in or after the month following your enrollment in this Health Insurance Society.

Health Checkup Course

• Change from gastric X-ray (barium) to gastroscopy (oral) will be subsidized by the health insurance society.
  If the gastric examination method is changed from oral to transnasal, there may be a cost difference. In that case, the full cost of the difference will be a copayment.
• Ewel General Health Checkup A1 Course for those who do not require a gastric examination requires examination with the abdominal ultrasound examination. (No copayment)
• Please receive either mammography or breast ultrasound.
  If you choose to receive both, the full cost of the breast ultrasound examination will be a copayment.
• The full examination costs of all tests ordered by a physician will be copayments.
• Copayments will be listed on the health checkup ticket (or completion e-mail in the case of smartphones) received from Ewel, Inc. and please check them.

Procedures

STEP 1	Access KENPOS and perform Initial Registration. Access KENPOS and register for the first time. * Please have your health insurance card ready.
STEP 2	Find a health checkup organization at which to receive a checkup Log in to KENPOS, press the Health Checkup Application/Procedure button to move to the application page, then select a health checkup organization from the Health Checkup Application/Issue Health Checkup Ticket button.
STEP 3	Call the health checkup organization directly to make an appointment. When making an appointment, be sure to provide the following information: Name of the health insurance society "Sanofi Health Insurance Society" The agency must be Ewel, Inc. Desired date of examination, health checkup course, and optional tests Your name, address, and contact information If you have any questions regarding the details of the implementation of the medical checkup, please consult the health checkup organization. Please schedule your visit at least 2 weeks ahead of your appointment date.
STEP 4	Request a health checkup ticket from KENPOS Log in to KENPOS and follow the procedure. By personal computer After the procedure, you will receive a health checkup ticket issuance completion e-mail from Ewel, Inc. within one week. Log in to KENPOS and print the health checkup ticket file. By smartphone After the procedure, you will receive a health checkup ticket issuance completion e-mail from Ewel, Inc. within one week. * If you applied by smartphone, no health checkup ticket will be issued, so store the email carefully.
STEP 5	Receive your health checkup On the day of your checkup, please bring the following items with you: Health checkup ticket Copayment amount (if paid over the counter) Health insurance card Specimen container, etc. (if received from the health checkup organization) If you applied by smartphone, please tell the health checkup organization reception counter your appointment number, as shown in the health checkup ticket issuance completion e-mail, instead of your health checkup ticket.

Changes and Cancellations

After notifying the health checkup organization of the change or cancellation, please check follow the procedure under Check, Change, or Cancel Application Details.
